Health insurance underwriting is the systematic process by which an insurer evaluates an applicant's risk, decides whether to offer coverage, determines under what conditions, and calculates the appropriate premium. At its core, underwriting exists for one reason: to keep the insurance pool financially stable. When premiums match expected claims, the insurer can pay benefits to sick policyholders using money collected from healthier ones. If the pool fills up with high-cost individuals paying below-market premiums, the entire system unravels.

The specific threat underwriting guards against is called adverse selection — the tendency for individuals who expect high medical costs to be more eager to buy insurance than those who expect to stay healthy. Imagine a world with no underwriting. A person diagnosed with cancer learns they need $400,000 of treatment. They rush to buy a comprehensive plan and pay one month's premium before claims begin. The insurer, unaware of the diagnosis, accepts them at a standard rate. That single enrollee's costs dwarf their premium contribution, and if many such individuals enter the pool, premiums must rise for everyone. How Health Underwriting Differs from Life Insurance Underwriting

Life insurance underwriting focuses on mortality risk — the probability that the insured will die during the policy period. Because life policies often run for decades and lock in premiums at issue, the underwriter's job is to estimate long-term life expectancy at a single point in time. Death is a binary, once-in-a-lifetime event, making aggregate mortality highly predictable through actuarial tables. The insurer has essentially one underwriting shot to price the risk correctly.

Health insurance underwriting focuses on morbidity risk — the likelihood that the insured will become ill or injured and use medical services, and how much that care will cost. Unlike mortality, morbidity is a continuous process. A single policyholder may generate dozens of claims in a year, ranging from a $15 prescription copay to a $250,000 hospital stay. This creates high-frequency, variable-severity claims that are harder to predict than mortality, particularly because they are influenced by medical inflation, new specialty drugs, and changes in how often people seek care.

The Regulatory Landscape: When Medical Underwriting Is and Isn't Used

A critical nuance for the exam: the Affordable Care Act (ACA) prohibits traditional medical underwriting for ACA-compliant major medical plans. Insurers selling individual market and small-group plans cannot deny coverage, charge higher premiums, or exclude benefits based on a person's health status or pre-existing conditions. In these markets, the underwriting function shifts almost entirely to eligibility verification and administrative compliance.

However, medical underwriting remains fully operational in several markets: short-term health plans, Medicare Supplement (Medigap) plans purchased outside guaranteed-issue periods, disability income insurance, long-term care insurance, and life insurance.
