Overview: The single most important — and most commonly misunderstood — limitation of Medicare is that it does not pay for long-term custodial care. Understanding this gap, and the tools that fill it, is the foundation of senior financial planning.

The Three Levels of Long-Term Care

Long-term care exists on a spectrum of medical intensity, and coverage rules differ dramatically across levels:

Skilled nursing care is the highest level of care available outside a hospital. It requires a physician's order and must be performed by or under the supervision of a licensed medical professional — a registered nurse (RN), licensed practical nurse (LPN), or licensed therapist. Examples include IV therapy, sterile wound care, ventilator management, and post-surgical physical therapy aimed at restoring function. It is typically short-term. Settings: skilled nursing facilities (SNFs), transitional care units, and certified home health agencies. Medicare covers this level — but only when medically necessary, following a qualifying hospital stay, and for a limited duration (up to 100 days per benefit period, with significant daily copays after day 20).

Intermediate care falls between skilled nursing and purely personal care. It serves individuals who are medically stable but have chronic conditions requiring regular nursing oversight — medication management, monitoring vital signs, periodic nursing assessment — combined with assistance performing daily activities. Licensed nurses supervise, and CNAs deliver much of the hands-on care. Settings: intermediate care facilities (ICFs), some assisted living communities with nursing tiers, and residential care homes. Medicare does not cover this level.

Custodial care is non-medical assistance with activities of daily living (ADLs): bathing, dressing, eating, transferring, toileting, and maintaining continence. It may also include help with instrumental activities (meal prep, housekeeping, medication reminders) and safety supervision for people with cognitive decline. No medical training is required to perform it. Settings: the person's home, assisted living facilities, memory care units, adult day care centers, and the residential side of nursing homes. Medicare does not cover this level. This is the category that devastates retirees financially — and it accounts for the vast majority of long-term care services consumed.
