Overview: Understanding the distinction between basic and major medical coverage is foundational — the two approaches embody opposite philosophies about which risks matter most and who should bear them.

Health insurance has always faced a fundamental design tension: do you protect people from the small, frequent, predictable costs of routine care, or from the rare but financially catastrophic costs of serious illness? Basic medical expense insurance and major medical expense insurance were historically the industry's two answers to that question, and understanding how they differ is the starting point for everything else in this chapter.

Basic medical expense insurance — sometimes called "first-dollar coverage" — is designed to pay common, predictable, low-cost medical expenses without requiring the insured to pay anything first. Because it pays from the first dollar of a covered claim, it typically carries no deductible and no coinsurance. The tradeoff is that its benefit limits are low and narrowly defined. Historically, basic coverage was divided into three separate buckets:

Hospital expense — covers room-and-board charges and routine hospital supplies during an inpatient stay.
Surgical expense — covers surgeon fees and operating room costs.
Physicians' (non-surgical) expense — covers routine office visits and non-surgical doctor consultations.

Each of these could be sold as a standalone policy or bundled together. The key structural feature is that reimbursement was — and in scheduled plans still is — based on a fixed dollar schedule: the policy lists a specific maximum dollar amount for each procedure, and the insurer pays up to that amount regardless of what the provider actually charges.

Major medical expense insurance takes the opposite approach. It is designed to shield the insured from catastrophic, high-cost medical events — the extended hospitalization, the major surgery, the intensive cancer treatment. It does this by covering a wide range of medically necessary services under one broad policy, with very high (historically millions of dollars, and now unlimited under the ACA) benefit limits. The structural cost-sharing tools it uses — deductibles, coinsurance, and stop-loss limits — are explained in detail later in this chapter.

The real-world consequence of this design difference is significant. Basic medical plans left people comfortable paying small routine bills but exposed them to financial ruin in a major illness — the scheduled benefit for a complex surgery might be $1,000 when the actual bill is $15,000. Major medical plans handled catastrophes but required people to self-fund initial costs through their deductible. The industry's eventual response was comprehensive major medical insurance, a hybrid that provides some first-dollar or low-cost coverage for preventive and routine care, then transitions into a major medical cost-sharing structure for larger events. Most modern health plans are comprehensive in this sense.
